A rare medical case reveals that intense joy can affect the heart as much as deep sorrow, after a 65-year-old woman suffered an unusual heart syndrome following overwhelming happiness during her daughter's wedding.

The phrase "I might die of happiness" has long been used to express extreme joy, but a recent medical study has shown that this phrase may contain a grain of truth, although this occurs in very rare cases.

A case study published in the Oxford Medical Case Reports documented the story of a woman who felt great joy after attending her daughter's wedding, before three days later she began to suffer from chest pain and shortness of breath, symptoms that led doctors to suspect she had suffered a heart attack.

Upon arrival at the hospital, tests showed a decrease in blood flow to the heart, a sign often associated with a blood clot, but doctors found no blockage in the arteries.

Instead, tests revealed an abnormal change in the shape of the heart's left ventricle, affecting its ability to pump blood efficiently. Following medical evaluation, the woman was diagnosed with Takotsubo cardiomyopathy, a rare condition that mimics a heart attack but is not caused by blocked arteries.

This syndrome typically occurs after experiencing strong, sudden emotions, whether negative, such as sadness and heartbreak, or positive, such as intense joy. The type associated with negative emotions is known as "broken heart syndrome," while the type resulting from intense joy is called "happy heart syndrome."

Although the syndrome is treatable, it is not a simple condition, as it can sometimes lead to serious, life-threatening complications. It is estimated that approximately 1 to 3% of patients suspected of having a heart attack but showing no signs of a blood clot may have Takotsubo syndrome.

Happy heart syndrome is much rarer than the type associated with sadness; it accounts for only about 4% of recorded Takotsubo syndrome cases where the emotional trigger is positive. Researchers believe the low number of cases may be partly due to the difficulty in diagnosing it because its symptoms are sometimes mild.

Takotsubo syndrome first appeared in 1983 at a hospital in Hiroshima, Japan, and got its name from the traditional Japanese octopus traps, "takotsubo," because of the shape of the left ventricle of the heart in those affected by it, which resembles those traps.

The precise mechanisms behind this syndrome are still not fully understood, but experts believe that a sudden increase in sympathetic nervous system activity and levels of stress hormones, such as catecholamines, plays an important role in its occurrence.

Researchers point out that the heart's response may vary depending on the nature of the emotions that caused the condition, explaining that happy heart syndrome may be associated with different patterns of heart changes compared to broken heart syndrome, and its symptoms may be less severe in some cases, but it needs further study.

Cardiologists called for increased awareness of this condition among doctors and patients, stressing the need to not rule it out even in people who do not have the usual cardiac risk factors, because intense emotions can affect the heart in unexpected ways.

In the case documented by the study, the woman made a full recovery and returned to her normal life, and the researchers said she is looking forward to sharing the next happy moments with her children.
