A major scientific review, involving more than 300,000 people and published in The Lancet Public Health, revealed that oral health and metabolic health are more closely linked than previously thought.
The study found that gum disease is associated with an increased risk of type 2 diabetes, and that the relationship works both ways.
Gum disease, also known as periodontitis, is a severe form of gum inflammation that damages the tissues and bone supporting the teeth, leading to swelling and bleeding, and potentially tooth loss if left untreated.
An international team led by Portuguese researcher Joao Botelho analyzed 28 studies from 16 countries to track the chronological order of the emergence of these two cases.
The researchers found that those with gum disease at the start of the studies were 18% to 25% more likely to receive a new diagnosis of type 2 diabetes during a follow-up period of 5 to 20 years. Complete tooth loss was an even stronger warning sign, with a 30% higher risk of developing new diabetes among those who had lost all their teeth.
Conversely, people with diabetes were more likely to develop gum disease and subsequently lose teeth, confirming a bidirectional relationship. This is because gum disease causes chronic inflammation that can enter the bloodstream, potentially interfering with the body's response to insulin and leading to insulin resistance, a hallmark of diabetes. In turn, high blood sugar weakens the body's ability to heal and fight infection, making people with diabetes more susceptible to oral problems.
However, the researchers acknowledge that their study was observational, not clinically proven, meaning it does not establish a definitive causal relationship but rather indicates a strong statistical correlation. Furthermore, diagnostic methods for gum disease were not standardized across all studies, and evidence regarding tooth decay and oral cancer was limited.
These findings come at a time when the world is facing huge numbers of people with diabetes (830 million) and oral diseases (3.7 billion), making the integration of oral health into diabetes prevention strategies an urgent necessity.
The researchers point out that dentists can play an important role in identifying and referring patients at risk of diabetes for screening, while diabetic patients can receive more regular oral health assessments.
The researchers conclude that while gum disease is not a diagnosis of diabetes, symptoms such as red, swollen, or frequently bleeding gums warrant a visit to the dentist, especially if accompanied by known risk factors for diabetes. The mouth may not offer a precise prediction of the future, but it certainly provides health indicators that should not be ignored.
